Check that your address is correctly shown on your proof form.
Important because this same address is used to deliver your
finished photos. If changes are needed, use the space provided in the lower left corner of your form.
When requesting the Commencement Photo Plaque, your name
will be engraved on the nameplate exactly as printed on your proof
form. You may specify a change in the
space provided. Photo plaques are delivered
approximately 10-12 days after any other photos
arrive.
Clearly indicate your choices for any quantity or
combination of photo packages
by indicating the package quantity (1,2,3, etc) in
the proper box.
Add the proper shipping/handling fee to arrive at the "Total
Amount". If you should have more than one order
form, one check may be written for for the
combined "Total Amounts".
Tennessee residents: Multiply the SUBTOTAL
amount by the Tennessee sales tax rate of 9.25%.
Make check payable to "GRADUATION FOTO".
We also accept money orders, credit or debit cards.
Late orders are always welcome. However, due to our
lab
minimums, the TOTAL AMOUNT ENCLOSED must be at least $20.00
